Persistent pain can be confusing. The injury may have healed, imaging may not explain the severity, yet burning, tingling, electric shocks, or deep aching continue to shape every part of the day.

For some people, the issue involves nerve injury or disease. For others, the nervous system itself has become unusually reactive. These patterns can overlap, and neither is a reason to dismiss what you feel. Pain is real, even when its cause is complex.

Neuropathic pain is pain caused by a lesion or disease affecting the somatosensory nervous system, the system that carries sensations such as pressure, temperature, and touch. A widely used clinical definition distinguishes this kind of pain from pain arising primarily in damaged tissue or inflammation, and from pain that cannot be fully explained by either process alone (neuropathic pain definitions (Treede et al., 2008)).

Common descriptions include:

Burning, prickling, or pins-and-needles sensations

Shooting, stabbing, or electric shock-like pain

Numbness alongside pain

Pain from light touch, clothing, or a gentle breeze

Symptoms that feel disproportionate to the original injury

These symptoms can occur after many different health events. They can also have different causes. That is why a label alone is not enough. We want to understand the pattern, its timing, your health history, and the ways pain is affecting sleep, movement, work, and connection.

What central sensitization means

Central sensitization describes increased responsiveness within the central nervous system, particularly the brain and spinal cord. In plain language, the pain system becomes more sensitive to incoming signals.

That does not mean pain is imagined. It means the system processing danger and sensation may amplify messages that previously would have been manageable, or would not have registered as painful at all.

Researchers describe central sensitization as a process that can contribute to heightened pain sensitivity, pain spreading beyond the original area, and pain responses to normally non-painful stimulation (central sensitization across pain conditions (Arendt-Nielsen et al., 2018)). It is a mechanism, not a diagnosis you can assign to yourself from a symptom checklist.

Someone with a sensitized pain system may find that a light touch hurts. A brief activity may trigger a longer flare. Stress, poor sleep, illness, or repeated overexertion may seem to turn the volume up. Still, these experiences are not proof of one single cause. A careful assessment remains essential.

Neuropathic pain and sensitization can overlap

It is tempting to put pain into neat categories. In real life, pain is rarely that tidy.

A peripheral nerve can be injured or irritated outside the brain and spinal cord. At the same time, ongoing signals from that area can change how the central nervous system responds. Reviews of peripheral and central mechanisms show that neuropathic pain may involve changes at multiple levels of the nervous system, rather than one isolated malfunction (peripheral and central pain mechanisms (Meacham et al., 2017)).

This overlap helps explain why two people with a similar injury can have very different experiences. One may recover steadily. Another may continue to have severe pain, altered sensation, sleep disruption, and fear of movement months later.

It also explains why we avoid simplistic conclusions. Persistent pain does not automatically mean central sensitization. Central sensitization does not rule out a treatable structural issue, nerve compression, metabolic condition, or inflammatory process. Both the body and the nervous system deserve attention.

Why an accurate pain assessment matters

The word “pain” covers many experiences. A muscle strain, arthritis, migraine, nerve injury, and complex regional pain syndrome can all hurt, but they may not call for the same clinical questions.

Clinicians often consider whether symptoms appear predominantly nociceptive, neuropathic, or related to central sensitization. That distinction can support clearer treatment planning, while recognizing that mixed presentations are common (clinical pain classification guidance (Nijs et al., 2015)).

A useful evaluation may include discussion of:

Where the pain began and whether it has spread

Sensations such as burning, numbness, coldness, or shocks

Injury, surgery, diabetes, infection, or other medical history

Changes in strength, balance, coordination, or bowel and bladder function

Sleep, mood, stress, medication use, and daily functioning

New weakness, sudden severe pain, fever, loss of bowel or bladder control, chest pain, or symptoms after a significant injury may require urgent medical attention. Do not assume persistent symptoms are simply part of sensitization.

The daily burden is more than physical

Chronic pain can narrow life gradually. You may stop driving after dark. You may avoid stairs, social plans, exercise, or the touch of someone you love because you do not know what will trigger the next flare.

Sleep is often affected. So are concentration and mood. This is not a character flaw or a failure to cope. The nervous system does not operate separately from the rest of your life.

Persistent pain often calls for patience, monitoring, and coordination with the clinicians involved in your care. Is central sensitization the same as neuropathic pain?
